Before your next appointment, please update the following in your portal: 

• -Sign the 2026 Practice Policies and Consent Form. This form MUST be signed every year. 

• -Medications & Supplements – Make sure your current list is accurate.

• -Insurance – Verify we have your current information.

• -Contact Info – Confirm your address and phone number.

• -Preferred Lab – Update if it has changed. 

• -Appointment Prep Form – Complete before each appointment with John Fallon, FNP.

• For new patients / initial consultations: 

• Submit all questionnaires, forms, and prior medical records at least 3 weeks before your first visit.
